If you are running a pet food store or shop, then this template could be used for easy invoicing. The form is printable on standard A5 paper.

This template assumes all the items are taxable. The formula of the tax cell is simplified, like this:
=ROUND(oknSubTotal*oknTax1Rate,2)
Another noticeable feature of this form is the additional "tax state name" option. It is a drop-down list. By default, the template has these state names listed: MD, VA, DE, NJ, PA, NY, CT, RI, MA, NH, and VT, with their corresponding tax rates filled out in the next column (see Section O3: P13). The tax rate cell picks up the correct rate value according to the state name you choose from the "tax state name". The formula looks like this:
