This is a variant of the Hourly Service Billing Form (c4025). It differs from the original template by adding a Tax column, assuming prices include taxes.
The tax on each item is calculated with a formula like this:
=round(if(oknTaxable_1, oknLineTotal_1 * (1- 1/(1+oknTax1Rate)), 0,2)
This formula means that, if the current item is taxable, the amount of tax will be "Line Total * (1-1/(1+Tax Rate))". For example, if the price is 10 and the quantity is 1, and the tax rate is 17.50%, then the tax amount equals 1.49 after rounding to 2 decimal digits.
