It is a service invoice template that does not include the shipping address fields in the printable form. The form adds several job-related fields, including job start and finish dates, work or job addresses, etc.
This template is based on the default template shipped with our invoicing app. The new fields like Start Date, Finish date, and Job Address are custom fields defined both on the database and template levels.
