This design has six columns in the detail section: Description, Date, Hours, Rate, and Amount. The Amount is calculated automatically using Excel formulas once the Hours and Rate are filled in. The other 5 columns, Description, Date, Hours, and Rate, are all editable.
As you can see from the screen shots, this is basically an hourly service template, like the Hourly Service Invoice Template (c4025).
On the top of the form, you see a logo image. This is a placeholder image that you should replace with your own image.
