This form is similar to the Generic Service Invoicing Template (c3002), but with the summary section ("Shipping and Handing", "Total", "Paid", "Total Due") moved to the top near the "Bill To" section.
It is ideal for those small service businesses, freelancers, or sole traders that don't need a shipping address section on their documents.
The form contains three columns: line number, description, and amount. The "Amount" field is manually editable.
