This template could be used by clinics such as dental clinics, cosmetic clinics, etc. It has fewer fields than the medical invoice template (c7019), but has a cuttable payment slip that copies the invoice number (receipt number), date, and amount due from the main part by using Excel formulas.
Note that although this template could be used for "Receipt" by changing some text labels on the form, the Excel worksheet name ("Invoice") shouldn't be changed. Our app depends on the sheet name to determine what to do when a button is clicked.
