This form itemizes the cost of materials and labor in two separate sections. Each section has its own tax rate and formula. It also features a bricklayer cartoon image that intuitively shows what your business is.
If you are in the construction or masonry (c5134) industry, your services may not be limited to building houses. You may also help to beautify the client's yard and landscape, etc. This billing form is suitable for all these situations.
On the top of the sheet, the business name, address, and contact details are right-aligned, leaving space for the bricklayer image. Under that are the required fields "invoice#" and "date".
On the "Services" section, the 4 columns are titled "Details", "Rate", "Hours", and "Amount". In the "Parts and Materials" section, the columns are titled "Details", "Price", "Quantity", and "Amount".
