This billing summary template was originally designed for an office of information technology that provides radio communication services. The downloadable package includes two files: the one-page template and the two-page template.
The usual "Bill To" section is now renamed "Agency". The labels in this section include "Account Number", "Name", "Department", "Contact or Operator", and "Vehicle Number". The last three fields are custom fields.
The 2-page template has 55 lines in the detail section, whereas the 1-page billing summary format offers only 19 lines. The 1-page template was created by simply hiding lines 20–56 of the 2-page template. Both templates connect to the same Microsoft Access database "c5122.mdb".
