This template is designed for television service providers. It is suitable for both satellite TV and cable companies. The format features fields like "Installer", "Account Number", a customer signature section, etc.
The "Bill To" section in the printable section works in a different way than most templates here on InvoicingTemplate.com. All fields in this section are locked, which means they are not manually editable.
The manually editable fields for the "Bill To" are placed at the left portion of the sheet, where all the fields related to a customer are editable, including "Category", "Customer#", "First Name", "Last Name", "Address", "City", "State", "Zip", and "Phone".
The "Shipping Address" section now has a new title, "Service Address".
On the bottom of the detail section, an Excel text box, namely "oknWidget_msg1", is added, which shows a fine print like this:

Another text box underneath it is a placeholder for your "TERMS OF AGREEMENT", which you should fill in before creating your own invoices.
The "Customer Sign" and "Date" on the bottom are designed for manually writing once the document is printed.
