This is a 4-column variant and an alternative version of the 2-column plumbing bill format. The four columns are Quantity, Description, Unit Price, and Line Total.
The original versions are Plumbing Contractor Invoice Template (c2008) and Plumbing Contractor Bill Template (Landscape) (c2009). The original forms were designed with VAT tax in mind. This sample changes it to sales tax, and the Taxable column is also moved into the printable section.
The "Taxable" column is a column of checkboxes. Each checkbox controls whether the item is taxable.
